     Please respond to this questionnaire and return this information to NAHIM by e-mailing this document.  The information you provide about your PHA will be very useful in determining the issues that should be covered in the NAHIM NEWS, and would be the key topics to be covered at the annual NAHIM Conference. The personal finance information you provide will be kept confidential.  The instructions on how to e-mail  your response back to NAHIM will be found at the bottom of this document.
